This template was designed to increase the usability and user feedback for online applications. Some of the features include: navigation bar fixes to the top of the page after scrolling below the header image, once all the required fields are filled out the circle beside the page name becomes a check mark, and easy recognition of required fields by using a background color. The template was converted to a fully functional online application upon completion of the design. The template can easily be updated by using html and css to match the look and feel of any school's existing brand.
